Start a new version when the effect definition, process boundary, category model, or material evidence changes. Do not overwrite rejected branches: retain their status and rationale so a later team does not repeat the same test. For printing, leave generous space around each rib and place the evidence register on a second page. The 6M fishbone diagram template is a memory aid, not a standard that must fit every domain. Rename “Manpower” to “People” when clearer, and use “Environment” for physical or operating conditions rather than dumping every uncategorized idea there. If two categories repeatedly contain the same mechanism, merge or redefine them. Category quality is measured by whether it prompts relevant, distinct questions—not by whether it preserves an acronym.
