Exclude “not applicable” only with a documented reason and reviewer. Report mandatory blockers separately; a high completion percentage cannot override a safety, legal, denominator or data-integrity failure.

The completion rate is 12 ÷ 20 = 60%, but the action is not “finish 40% and launch.” The safety signal escalates immediately; denominator and taxonomy controls must be repaired; remaining evidence gaps receive owners.

A checklist improves consistency by making evidence and ownership visible. It cannot transform weak evidence into a root cause. Read failed and needs-input items by severity and dependency. Preserve contradictory evidence and reviewer limits instead of forcing every row to pass.
